Fix leaks in tests (#2295)
Generally, a test fixture in a method that can generate a binary
should release any previously cached binary. Similarly for diagnostics.

Cleanup includes. (#1795)
This Cl cleans up the include paths to be relative to the top level
directory. Various include-what-you-use fixes have been added.
