11a867f412 Add FlattenDecoration transform 
Add --flatten-decorations to spirv-opt

Flattens decoration groups.  That is, replace OpDecorationGroup
and its uses in OpGroupDecorate and OpGroupMemberDecorate with
ordinary OpDecorate and OpMemberDecorate instructions.
