Add name annotations to the generated instrumentation code to
make it easier to understand. Example spirv-cross output:

spirv-opt: Set parent when adding basic block ( #4021 )

Ensures that the parent of a block is set in Function::AddBasicBlock.
Removes various now unnecessary calls to BasicBlock::SetParent.

Check feature mgr in context consistency check ( #2818 )

We add a check that the feature manager is correcter after each pass.
This resulted in a couple failing tests cases. Those are fixed.
