Ehsan Nasiri aadf696fce Fixing bug in OpAccessChain validation code. 
The validation code for OpAccessChain was missing OpTypeRuntimeArray as
a possible type that can be indexed into.

This was caught by running the validator on VKCTS.

Also adding unit tests for it.
