Adam Sawicki 3c6470cf45 New JSON dump format 
Unified across VMA and D3D12MA.
Updated Python script for visualization - now called GpuMemDumpVis.py.

Also a fix for bug in EXTENSIVE defragmentation algorithm - see #232
