[cid, truetype] Don't use `index' as a variable name.
At least on FreeBSD there is a global declaration of `index' in file `/usr/include/strings.h'. * src/cff/cf2intrp.c, src/truetype/ttgload.c: s/index/idx/ where appropriate.
