Reduce the number of unwanted exports

Nuke GailAdjustment
This was a fairly trivial wrapper around GtkAdjustment with
no particularly important reason for existence.

First cut at GailEntry -> GtkEntryAccessible
Replace the factory for entry and spin button with
gtk_widget_class_set_accessible_type() calls, and move then
to the GtkAccessible namespace.
