Doc: Fix QSql*Model snippets

- QSqlQueryModel docs contained a snippet about QSqlTableModel.
- Snippet #25 was about QSqlTableModel, but it was previously unused.
- This patch ensures that snippet code matches the corresponding text
  descriptions.
