I didn't even try to understand what the old code was trying to do;
once you're told by a user that the code is wrong, you see that it is.
Fixed by just using the row as passed to takeRow() instead of trying
to do some storage-index calculations. The m_matrix indexing operator
does it all for us.
Added a test that checks that the expected field widget gets
returned. Fixed expected test data that was wrong, and just checking
that the implementation behaves as implemented, instead of as
documented.
Amends change 8fbae648db.

The code coerced a -123 into a QFormLayout::ItemFlags, which, however,
being an enum with enumeration values 0..2, only has valid numerical
values 0..3.
Fix by using 3 as the value to represent the invalid enum value, and
store this in a constant so as not to distribute this magic number
all around the test class.

The new takeRow() functions return a pair of pointers to
QLayoutItems and, as the name particle 'take' suggests,
releases ownership of these layout items. Which in turn
means that the caller of the function is supposed to deal
with them.
This was not done here.
To fix, write a RAII class that takes ownership of the
returned layout items, deleting them when it goes out of
scope or gets a new value assigned (only move special
member functions are implemented, making the class move
-only).
Deleting the QLayoutItems is not so easy, though:
QFormLayout has a special function for clearing the
QLayoutItems out, so it appears that just calling their
destructors is not going to fly (though I don't know off
the top of the head why that should be a problem).
Solve this, for now, by adding the layout items back into
a temporary QFormLayout for destruction.

QFormLayout currently allows to remove a row only through takeAt, which
doesn't resize the internal structure used to store the layout-related
data. The implementation of rowCount uses that structure so it returns
a row count that doesn't match what the user sees.
The removeRow methods complement that be doing a "real" removal which
will also keep the row count in sync.
[ChangeLog][QtWidgets][QFormLayout] Added removeRow(), takeRow().

Widgets and layouts added or inserted to a layout are checked for:
- Not being NULL
- Not being the parent widget of a layout or the layout itself,
respectively
Without this commit, adding a widget to its own layout would result in a
CPU-hogging infinite loop. Now, a warning is written to stderr and the
add or insert function call is ignored.
The checks are implemented as public functions of QLayoutPrivate and
thus accessible in QLayout's descendants to be used in various
"addWidget", "insertWidget", etc functions.
Unlike 'classical' layouts like QGridLayout, QFormLayout does indeed
accept widgets that are NULL. To not break this behavior, any call for
the check functions first tests if the widget or layout, respectively,
to test is NULL or not and calls the check only in the latter case.
Automated tests for QBoxLayout, QGridLayout, and QFormLayout were added.
For an unpatched Qt 5.3, each of those automated tests will freeze as
explained in QTBUG-40609. For a fixed version, warning messages about
invalid parameters to addWidget/addLayout/... calls will be read by
QTest::ignoreMessage, resulting in a passed test.

QStyle::pixelMetric(PM_Layout{Vertical,Horizontal}Spacing), but both
QBoxLayout and QGridLayout seems to be robust enough to handle this.
They will simply make sure that the spacing is never negative.
We therefore make QFormLayout equally robust.
