It was too easy to get into circular include chains. Added static
asserts to ensure we keep our quad AA flags in sync. Also, IWYU.

Adds SkRuntimeEffect, a ref-counted object to hold SkSL,
a Compiler, and compiled Program, along with uniform info.
Removes the old SkSLFP factory and factory cache types.
Caching is done by simply holding on to the effect (or
the shader/color filter factories, which own an effect).
The effect class is simply ref'd by the factories, the
shader and color filter types, and the FP.
NOTE: Changes to public headers only affect private functions.

Current strategy: everything from the top
Things to look at first are the manual changes:
- added tools/rewrite_includes.py
- removed -Idirectives from BUILD.gn
- various compile.sh simplifications
- tweak tools/embed_resources.py
- update gn/find_headers.py to write paths from the top
- update gn/gn_to_bp.py SkUserConfig.h layout
so that #include "include/config/SkUserConfig.h" always
gets the header we want.

Both GrContext and GrContextThreadSafeProxy had their own copies. This centralizes ownership and standardizes how all the contexts get initialized.

The GrContext's ID isn't really unique any more (since it can be shared among a family of contexts). Change its name to reflect the new reality.
Additionally, no client seems to be using it so make it private.
