Mike Klein b5c435579e upgrade debugging tools
- Add instruction numbers to program dumps.
- Dump the program when an assertion fails,
and print the failing condition or an optional
other value (e.g. if alpha outside [0,1], print alpha).

With all that and the new commented assert enabled, I'm seeing that
sometimes we get a bilerp alpha of 0x3f800001, just a little more than
1.0f. Fix still tbd.
