This moves the code to allocate the far jump table from {SetRuntimeStubs} to {AddCodeSpace} to allocate one such table per code space. Also, the {runtime_stub_table_} and {runtime_stub_entries_} fields do not make sense any more now and are replaced by calls to {GetNearRuntimeStubEntry} and {GetRuntimeStubId}.
