Other functions that take (argc,argv) arguments (wxEntry, wxEntryStart)
take argc by reference, because they may manipulate the arguments list.
wxInitialize() used passing by value, so any modifications would be
silently lost. Make all the functions consistent in their handling of
argc by using int& everywhere.

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

Make wxEntry(HINSTANCE, ...) available in wxGTK under Windows too.
Refactor the headers to allow this and extract Windows-specific wxEntry()
declarations in wx/msw/init.h from wx/msw/app.h for consistency with the
normal wxEntry(int, char**) declared in wx/init.h.

Before with only include wxchar.h it was missing ATTRIBUTE_PRINTF and other things due to wxchar.h not including
defs.h because of defs.h including wxchar.h itself.
We probably don't need to include wxchar.h anymore in init.h then, but I don't want to rely on wxchar.h being
always included from defs.h (if we can rely on that, please change if appropriate)
