This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.
Closes#14487.

This should ensure that the entry is always valid, removing the need for r74315.
Also simplify DoInsertItems() by using DoInsertItemsInLoop().

Reuse wxListBoxBase::SetEvent() instead of duplicating its code in wxGTK.
Also get rid of the code checking for selection of the item with index -1:
this can't happen any more since r65865 which changed GTK_SELECTION_SINGLE to
GTK_SELECTION_BROWSE.

Bring wxGTK in line with wxMSW behaviour and avoid sending the
wxEVT_COMMAND_LISTBOX_SELECTED events when the user clicks on an already
selected item.
Refactor wxMSW code to extract the logic to avoid such events into a reusable
in other ports wxListBoxBase::DoChangeSingleSelection() function. Also add
wxListBox::GTKOnSelectionChanged() to wxGTK to avoid having to make the new
function public just so that it could be called by GTK callback and make the
previously existing CalcAndSendEvent() protected as well.
This fixes a unit test failure in ListBoxTestCase::ClickEvents() under wxGTK.

1. the main function for item insertion is now DoInsertItems() which allows
for much more efficient addition of many new items at once
2. the items client data management is done entirely in wxItemContainer
itself, the derived classes don't have to distinguish between void and
object client data
3. many fixes for sorted controls, in particular implemented wxCB_SORT support
in wxGTK combobox

- Introduced GTKGetWindow() which returns all GdkWindows associated with the
given wxWindow
- Renamed IsOwnGtkWindow() to GTKIsOwnWindow() to avoid confusion with the old
virtual function (the new one is non-virtual and is implemented in terms of
GTKGetWindow())
- Refactored/simplified event handlers code in gtk/window.cpp
- Fixed some header guards names (__GTKFOO__ -> _WX_GTK_FOO_H_)
- Added GTKUpdateCursor() which sets the current cursor for all windows
returned by GTKGetWindow()
- Factored out code from many different classes in wxControl::OnInternalIdle()
which now updates the cursor, checks for internal focus and sends update UI
events

from derived classes
2. made wxItemContainer::Select() non virtual: it simply calls SetSelection()
3. renamed wxListBox::SetSelection(n, select) to DoSetSelection() for all
ports and defined non virtual SetSelection() overloads in the base class
to avoid virtual functions hiding

1. don't modify the style unless something changed
2. use gtk_widget_modify_style instead of gtk_widget_set_style, the latter is not recommended by GTK+ developers

GNU compiler included with Mac OS X 10.2 (Jaguar) as well as August Developer
Tools update contain a bug concerning #pragma interface handling that can only
be worked around by not using them (and they are not necessary anyways)

prevent "hanging" in single selection mode.
Also removed call to AddPendingEvent and
changed wxLB_EXTENDED mode so that no changes
to the listbox go unnoticed.
I do wonder what the difference between multiple
and extended list boxes actually are.
Not suer what I changed in the other files.

2. to support this, new class wxControlWithItems added (ctrlsub.h/cpp) and the
controls sample modified to test it

Corrected cursors - not yet perfect,
Added some tests to scroll sample
Made wxScrolledWindow::Scroll() call Refresh(),
Change wxListCtrl to reflect the latter change (wxTreeCtrl will follow),
Added activate event for wxMDIChildFrame
Implemented wxSIMPLE_BORDER for wxWindow (no controls),
Tried to correct positioning upon start-up - no way,
Rewrote catching of scroll events - page-up, page-down, up and down
work now. Don't know what TOP and BOTTOM are used for.
