Don't claim that SetPriority() can only be called before the thread is
created, this is just wrong.
Do document that it must be done after creating the thread in wxMSW (this is a
limitation of this port as POSIX implementation allows calling it at any
moment and could be lifted in the future).

This compiler is not being developed since several years and almost certainly
can't be used to build the current wxWidgets sources anyhow, so remove all
support for it, including a lot of extremely ugly workarounds for its bugs
with template functions.

This port is not used and is not being worked on, so remove it to reduce the
amount of the code which needs to be updated for every global change.
Also remove tests for VisualAge compiler which isn't used since ages.

Under MSW a thread has both an ID, returned by wxThread::GetId(), and a
handle, which couldn't be retrieved so far. Add an accessor to do it.

Add Wait() overload that takes a functor argument and doesn't return
until the condition is signaled _and_ the predicate returns true. This
is useful for dealing with spurious wakeups and is modeled after C++11
std::condition_variable's corresponding method.

This keyword is not expanded by Git which means it's not replaced with the
correct revision value in the releases made using git-based scripts and it's
confusing to have lines with unexpanded "$Id$" in the released files. As
expanding them with Git is not that simple (it could be done with git archive
and export-subst attribute) and there are not many benefits in having them in
the first place, just remove all these lines.
If nothing else, this will make an eventual transition to Git simpler.

This fixes compilation of non-wxOSX ports (e.g. wxGTK) under OS X.
Also make the difference between the two symbols more clear in the
documentation.

This compiler doesn't exist any more and was probably unsupported even in 2.8,
let alone 2.9, so remove all the __MWERKS__ tests to simplify things.

This change prepares the way for using wxGTK under Windows as this would
still define __WINDOWS__ but use __WXGTK__ instead of __WXMSW__.

Add "wait mode" parameter to these methods which can be used to make them
block even under wxMSW where they currently dispatch messages when called
which can be totally unexpected.
Do keep the old behaviour for compatibility however, although it will change i

Call OnXXX() from wxThread::Kill() and Delete() respectively to allow the
thread being terminated perform some cleanup.

This is useful for checking if a message was logged from the main thread or not and also allows us to implement IsMain() by comparing GetCurrentId() with GetMainId() in all ports and avoid repetition.
