Note to reviewers: Start with changes to SkPDFTypes.h
Many places that had a bare pointer owning a reference are refactored to
use a sk_sp.
There remain several places where a non-owning pointer `T*` should be
replaced with `const sk_sp<T>&` to eliminate the common pattern
`sk_sp<T>(SkRef(x))`.

SkUtility.h and SkTLogic.h implement a number of type traits now
available through <type_traits> and <utility>. This removes SkUtility.h,
replacing it with <utility>, and moves a number of traits in
SkTLogic.h to use the std:: equivelents. This change only uses C++11
parts of the standard library; SkTLogic.h will continue to provide
C++14 and beyond for now in the skstd namespace.
The changes to SkTLogic.h are being done gradually so that safe changes
may be landed confidently, with more risky changes in the future.

The "zeroPath" and emptystroke GMs capture this issue.
This CL changes the following PDF GMs: emptystroke dashing4
lineclosepath dashing3 zeroPath linepath
complexclip3_complex complexclip3_simple roundrects
degeneratesegments filltypes strokerect pathfill
inverse_paths desk_chalkboard.skp
After this change, all PDF GMs look better (closer to 8888).
The dashing4, emptystroke, and zeroPath GMs still need a lot
of work to make them look right.

drawImage calls now properly embeds the original jpeg.
NOTE: drawBitmap*() calls no longer embed JPEG files when
possible (this is in advance of eliminating bitmaps backed
by encoded data). Chromium has already moved from
drawBitmap to drawImage.

control:
total PDF drawImage/drawBitmap calls: 8010
total PDF jpeg images: 0
total PDF regular images: 3581
experiament:
total PDF drawImage/drawBitmap calls: 8014
total PDF jpeg images: 271
total PDF regular images: 3311
total PDF regular images: 3582 (271 + 3311)
When comparing rendered output there were perceptual
differences in the following four GMs: colorcube, emboss,
colormatrix, and tablecolorfilter. All of these differences
were improvements (that is, closer to the 8888 rendering)
due fixing a bug with colorfilters and forgetting to call
notifyPixelsChanged.
No SKPs had perceptual differences.
Total PDF size dropped from 133964 kB to 126276 kB, a 5.7%
improvement (mostly due to restoring use of JPG images in
SKPs).

Previously annotations added between saveLayer/restore were lost.
Merge annotations in SkPDFDevice::drawDevice(). Also modified code to
apply correct transformation and clipping on annotations added between
saveLayer/restore:
- Apply the initial transform only when adding the annotations into the
doc, otherwise we need to unapply sub-device's initial transform
before merging the annotations into parent-device.
- Apply only device-local clipping. fClipStack is in global coordinates,
which is not suitable to clip rects in sub-devices.

This CL starts the process of pushing kLegacyFontHost_InitType-type SkSurfaceProps up the call stack and out of Skia. It:
Gets rid of the default SkBaseDevice ctor. This means everyone has to always hand an explicit SkSurfaceProps to it.
It makes public the SkBitmapDevice creation methods that require SkSurfaceProps.
Removes (in Skia's code base) all SkBitmapDevice ctor calls w/o SkSurfaceProps.
Makes the "recording" canvases (e.g., pdf, svg, xps) explicitly not use kLegacyFontHost_InitType.

In Release, running `dm --src skp --config pdf`, I get a
speedup of about 1.2%.
SkPDFGraphicState class:
- Holds the subset of SkPaint that maps to a PDF Graphics
State
- These fields are easily comparable, making hashtable
comparisons easy.
SkPDFCanon:
- findGraphicState() takes a SkPDFGraphicState, not a SkPaint
- fGraphicStateRecords is a SkHashSet, not a SkTDArray
SkPDFGraphicState:
- mode_for_pdf() replaces logic inside equivalent(), but is
only called once per lookup.
- emitObject() no longer modifies the SkPDFGraphicState to
cache the SkPDFDict stucture. (Since it is de-duped,
this get no speedup).
- Static Functions that don't use the canon return a plain
SkPDFDict now. No need for fPopulated.

- SkHashSet::forall added
SkPDFDevice; SkPDFShader
- Updated for new SkPDFGraphicState interface.

catalog.
- Modify SkPDFObject::emitObject and SkPDFObject::addResources
interface to requiore SkPDFObjNumMap and SkPDFSubstituteMap.
- SkPDFObjNumMap const in SkPDFObject::emitObject.
- Remove SkPDFCatalog.cpp/.h
- Modify SkDocument_PDF.cpp to use new functions
- Fold in SkPDFStream::populate
- Fold in SkPDFBitmap::emitDict
- Move SkPDFObjNumMap and SkPDFSubstituteMap to SkPDFTypes.h
- Note (via assert) that SkPDFArray & SkPDFDict don't need to
check substitutes.
- Remove extra space from SkPDFDict serialization.

- Make use of SkHashMap for lookups (simplifies code).
- inline all small methods
- emitXrefTable moved to SkPDFDocument.cpp
- no longer store offsets in this data structure (moved to
SkPDFDocument.cpp)
- setFileOffset gone.
- own substitute refs directly.
SkPDFDocument::EmitPDF()
- All sites that call into SkPDFCatalog modified.
- catalog.addObject only called in a single place, after the
resouceSet is built
- offsets moved to local array.
SkPDFPage:
- finalizePage no longer deals with SkPDFCatalog or resource sets.
- GeneratePageTree no longer deals with SkPDFCatalog
SkPDFObjRef
- emitObject respects the substitution map
Unit Tests:
- respect SkPDFCatalog::addObject signature change.

Motivation:
PDFDevice currently relies on 1) being told that the layer's paint has an imagefilter, and in the case, it creates a rasterdevice. It then relies on (2) canvas itself sniffing the layer's paint and offering to apply-the-imagefilter to call drawSprite instead of drawDevice.
This subtle interchange is fragile, and also does not support other unsupported PDF features like colorfilters. This CL is a step toward making this use-raster-instead-of-native approach to layers more completely in the subclass' hands.

Add SkPDFCanon::reset function to unref all objects.
No longer possible to remove object from canon
Motivation: this doesn't change these object's lifetime, (they will
still be fully unrefed when SkDocument::close() is called, but we no
longer have to remove them from the array when their destructor is
called.

- Adds miniz.c v115_r4 (latest release) to third_party.
- Merges SkDeflateWStream into SkFlate so including "miniz.c" links
without duplicating symbols.
The only interesting code change I've made is to remove the line
fImpl->fZStream.data_type = Z_BINARY;
from SkDeflateWStream::SkDeflateWStream(). miniz doesn't have Z_BINARY
defined, and as far as I can tell, both zlib and miniz ignore data_type.
We should be able to swap skflate.gyp's dependency between zlib.gyp:zlib and
zlib.gyp:miniz at will (except of course on Windows) if we're interested in
zlib itself. I've left android framework on its own zlib. I think this all
means we can stop defining SK_NO_FLATE on Windows.
I'll leave the possible cleanup of SK_NO_FLATE itself for another time. Might
be we always want to keep this dependency optional.

The PDF canvas is now just as threadsafe as any other Skia canvas.
DM updated to thread PDF tests.
SkDocument_PDF now owns SkPDFCanon, and pointers to that canon are
passed around to all classes that need access to the canon.

All image compression currently uses (losseless) Deflate, not Jpeg.
All clients simply use SkDocument::CreatePDF(stream).
SampleApp and SkLua still use SkDocument::CreatePDF(path).

Also: Add SkDeflateWStream and associated unit tests.
SkPDFBitmap is a replacement for SkPDFImage. As of now, it only
supports 8888 bitmaps (the most common case).
SkPDFBitmap takes very little extra memory (aside from refing the
bitmap's pixels), and its emitObject() does not cache any data.
The SkPDFBitmap::Create function will check the canon for duplicates.
This can reduce the size of the output PDF.
Motivation: this gives another ~40% decrease in PDF memory overhead
TODO: Support other ColorTypes and scrap SkPDFImage.

The bounding box used for our pattern sizing is in device coordinates,
but SkPDFImageShader::Create() runs it through the inverse CTM to map
into shader space (with a notable effect of vertical flipping).
In order not to implode, the fallback shader path needs to compute its
adjustments in a consistent space.

Prior to this change, SkPDFObject subclasses were required
to track their resources separately from the document
structure. (An object has a resource if it depends, via an
indirect reference, on another object). This led to a lot
of extra code to duplicate effort. I replace the
getResources() function with the much simpler addResources()
function. I only define a non-trivial addResources() method
on arrays, dictionaries, and indirect object references.
All other specialized classes simply rely on their parent
class's implementation.
SkPDFObject::addResources() works by recursively walking the
directed graph of object (direct and indirect) references
and adding resources to a set. It doesn't matter that there
are closed loops in the graph, since we check the set before
walking down a branch.
- Add SkPDFObject::addResources() virtual function, with
four implementations
- Remove SkPDFObject::getResources() virtual function and
all implementations.
- Remove SkPDFObject::GetResourcesHelper()
- Remove SkPDFObject::AddResourceHelper()
- In SkPDFCatalog::findObjectIndex(), add an object to the
catalog if it doesn't exist yet.
- SkPDFCatalog::setSubstitute() no longer sets up resources
- SkPDFDocument.cpp no longer needs the Streamer object
- SkPDFDocument.cpp calls fDocCatalog->addResources to build
the resource list.
- SkPDFFont::addResource() removed
- All SkPDF-::fResource sets removed (they are redundant).
- removed SkPDFImage::addSMask() function
- SkPDFResourceDict::getReferencedResources() removed.
Motivation: this removes quite a bit of code and makes the
objects slightly slimmer in memory. Most importantly, this
will lead the way towards removing SkPDFObject's inheritance
from SkRefCnt, which will greatly simplify everything.
Testing: I usually test changes to the PDF backend by
comparing checksums of PDF files rendered from GMs and SKPs
before and after the change. This change both re-orders and
re-numbers the indirect PDF objects. I used the qpdf
program to normalize the PDFs and then compared the
normalized outputs from before and after the change; they
matched.
