Don't allow creation of mip-mapped textures when caps says we don't
support mip-mapping.
Skip testing of mip-mapped resources in the resource size test,
when creation will fail.
For iOS devices with ES2, the APPLE BGRA8888 extension is more
trouble than it's worth. Even though it lets the internal and
external formats not match, it appears that the driver remembers
the first external format, so subsequent attempts to upload with
the other swizzle will fail. Up until now, creation of these
textures was failing anyway, so now just make it more explicit
that we don't support BGRA in this situation.
Now that we're testing our TexStorage path, fix a few other problems
that showed up:
- Command Buffer doesn't like extension formats.
- X86 PowerVR and Adreno devices don't like BGRA.

Combine texture provider and resource provider
Largely mechanical. Only three places that were calling createApprox

This also makes the required changed to src, tests, and tools. The few
public APIs modified by this change appear to be unused outside of Skia.
Removing these from the public API makes it easier to ensure users are
no longer using them.

Use DEF_GPUTEST_FOR_*_CONTEXT macros to obtain the
test GPU context.
Makes changing the context -related classes easier,
since not all tests need to be changed.

That way when declaring a test with DEF_TEST() macro, you don't have to
uniquify the test name because it might colide with the class it is
being testing.
For example, if you are testing SkBase64 and do:
DEF_TEST(SkBase64, reporter) {
}
That will generate an error because the macro will declare a function
named SkBase64 which colides with the type SkBase64.
By adding Test to the function name we avoid this problem.
Fixed the entries found with the following command line:
